Recurring Payment Authorization Form

Print out the forms needed, fill them out, and turn them into the Church office ONLY!

You are allowed to fill them in electronically with a PDF editor/signer such as Adobe Acrobat, DocuSign, etc, and print them out as well to turn into the Church office

To print these forms out, click the form needed, click the "File" button on the top left of the Google Doc then click "Print"
